This will allow you to easily input new data for each patient without having to recreate the body of the form each time. It has the capacity to greatly improve the efficiency of your office's workflow. No special software is needed--the standard version of Microsoft Word has this capability built in.

Open the form you wish to convert to a template in Microsoft Word.
If you are using Microsoft Word 2007 or later, press the Microsoft Office button and choose "Save As." If you are using an earlier version of Word, choose "Save As" from the "File" menu.
If you are running Windows Vista and using Word version 2007 or later, select "Favorite Links" and then "Templates." If you are running Windows XP and using Word 2007 or later, select "Save In" and then "Trusted Templates."
Skip this step if you are using an earlier version of Word. The template will be saved by default in the Templates folder.
Select "Word Template" or "Template" (depending on your version of Word) from the "Save as type" list.
Enter a name for your template in the "File Name" field and click "Save."
