If you're unhappy with your appearance, or know someone who is, consider nominating yourself or someone else for ABC-TV's "Extreme Makeover." The intent of the program is to alter the appearance of deserving people by providing them with physicians, plastic surgeons, fitness experts and others, along with the work and expenses required to make complete transformations possible. Follow a few simple steps, and dreams could come true.

Go to ABC-TV's website and look for the link to "Extreme Makeover." There you will find the written application, which you can download and print out, and the show's rules. If no shows are scheduled and you can't find the application, be patient. It will be posted online once a new season is under way.
Be prepared to reveal a lot about yourself. You will be asked things such as what areas or parts of your body you like or dislike and why. You will also be asked about your marital status, children, whether you have ever been convicted of a crime, had any restraining orders against you and other private details.
Take two photographs of yourself and have them printed. Make sure that they are labeled on the back with your name and your city/region number. One photo must be a close-up of your face, and the other must be a full-body picture.
Fill out the Application
Make a three-minute videotape of yourself, or have a friend do it, using a VHS tape. This is probably the most important aspect of applying to be on "Extreme Makeover," and the producers want to know more about your looks and your personality. They also want to know what makes you different from other applicants. You are encouraged to be creative.
Make sure your videotape includes a head to toe look at you, and describes what you want to change about your physical features.
Include close-ups of your face, your profile and your body. If you have complexion or teeth problems, make sure that the videotape honestly shows those problems. The producers ask that close-ups last for at least 30 seconds.
Watch the videotape before you send it in, to ensure that the lighting and sound are good. You don't want the tape discarded just because the producers can't hear what you're saying or can't see your features well enough to determine if you're a deserving candidate for the show.
Make a Video of Yourself
Make sure your application is complete before going to the post office. The application package must include the written application, the videotape clearly labeled with your name and city/region number on the face of the tape and the spine, the 2 color photographs of yourself and copies of 2 forms of identification (one must be a driver's license, the other must be one of 3 forms of ID--social security card, passport or birth certificate).

Find your region number, which is available on the website and is assigned to the major city that is nearest to you. You simply choose the city that is closest to you from the following list and the corresponding number: 01, New York City; 02, Baltimore; 03, Orlando; 04, Atlanta; 05, Indianapolis; 06, Minneapolis; 07, Louisville; 08, St. Louis; 09, Dallas; 10, Denver; 11, Seattle and 12, Los Angeles.

Tips and warnings
- Before applying for "Extreme Makeover," read the website and the fine print thoroughly to make sure you would qualify. For example, you must be a U.S. citizen, at least 21 years old, free of felony and misdemeanor convictions and you must not have appeared in a prime-time TV reality or game show series in the past.
- You must be able to commit the time it takes to tape "Extreme Makeover." You will be expected to be available for several days for the makeover process, and another four to seven weeks afterward for recovery and the big "reveal."
